ECHO Communication and Academic Performance Questionnaire

The Carle ECHO program wants to better understand the communication and academic performance of children served at Carle and other satellite locations. This questionnaire is for parents or legal guardians of children who are deaf and hard of hearing. Through this questionnaire, the ECHO team would like to better understand the child’s communication skills and performance at school, as well as any challenges they might face in their learning. Please answer the following questions, it should take about 10-15 minutes.
